We are required by the state to have certain forms in your child’s folder. If your child needs a well check, what we need is a statement from your child’s doctor that states they have been examined in the last year that they are in good health and can participate in school. THIS IS NOT A COPY OF THE SHOT RECORD If your child is listed as needing a shot record or updated shots…this is a STATE requirement, and children will not be able to attend school without these records in their file. If you child is needing updated paperwork, please complete and send back.
